This is a Schurter 3404.0016.22 surface mount fuse from the OMF 125 series. It is a quick-acting (fast blow) fuse rated for 4A at 125VAC and 125VDC with a breaking capacity of 100A.
The fuse has a melting I²t of 2.1 A²s and operates over -55°C to 125°C. It is housed in a 2-SMD J-Lead package measuring 7.40mm x 3.10mm x 2.60mm. The terminals are tin-plated copper alloy.
Designed for direct soldering on PCBs, it supports reflow and wave soldering. It is cURus approved and meets UL 94V-0 flammability requirements. Moisture sensitivity level is MSL 1.
